     On most cars it means the module went to "Sleep" due to inactivity.  But please, try to provide a little more info of the year make and model otherwise most people won't bother to reply.

     Honda calls it APP Sensor(aka Throttle Body)
    Both L4 & V6 Motors have a six wire connector at the throttle body(The functions of the terminals are the same just the colors and terminology change between the 4 & 6 cylinder engines. 
    I did not check the Hybrid version as I don't know which engine you are working on.
    My best guess is that you have an open wire form the ECM to the Throttle Body(Or A Bad Throttle Body Altogether-Not Common but anything is possible)
    Back to your question, what I thing is happening is that the ECU wants to "See" a voltage change when it run the Throttle Body thru its opening and closing range. If you have an open wire - hence the scan tool will think it is not able to close the "Relay" or power circuit.
    I would start by checking the wiring between the Throttle Body To ECU. If you need wiring diagrams I may be able to provide them.
