Can you command the injector off with your scan tool? If not check the pulse signal
wire from injector to pcm connector for a short. If it tests fine then replace PCM

Hi, white smoke from exhaust can be caused by several reasons,check your fuel for
contamination like diesel fuel in the gas,also when it consume oil or engine coolant
coming through gaskets,you can start by changing the injector.
